Primary Care Provider

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. The Primary Care Provider is an integral member of the Primary Care Service Line who is assigned to both PACT panel management and GAP coverage based on clinical need and service line scheduling. Duties as a PACT Provider:

• Participates in daily huddles with other PACT team let staff (nurse care manager, clinical support staff, pharmacy, behavioral health, etc.) and contribute to primary care systems redesign projects and continuous quality improvement initiatives.
• Manages a panel of patients in the outpatient setting under the Primary Care Management Model (PCMM) guidelines. The recommended panel size at the division level is calculated based on a standardized formula. Each provider's panel size is specific to their practice setting and clinical context.
• Delivers comprehensive clinical care for patients on their assigned panel. This includes management of acute and chronic conditions, medication management, preventative care and management of clinical reminders, timely management of clinical alerts, home care orders, telehealth services, and addressing other healthcare needs of the patient. The provider is expected to assist with coverage of other department member panels during absences or based on clinical workload demands.
• Responsible for clinical oversight, consultation, and collaboration with primary care Advanced Practice Providers (APPs) and other clinical staff as assigned.

GAP responsibilities include:

• Provides acute and same-day access for Veterans who cannot be seen by their assigned Primary Care Provider (PCP).
• Manages walk-ins, urgent care needs, and time-sensitive clinical issues based on clinic demand and triage protocols.
• Reviews charts and completes medication renewals, laboratory follow-up, and consultation management for absent providers when clinically appropriate.
• Assists with coverage during unexpected staff absences, leave, or vacancy periods to maintain continuity of care operations.

Telehealth & Virtual Care:

• Utilizes VA virtual care platforms including VA Video Connect, MyHealtheVet, Secure Messaging, telehealth consultations, and telephone visits as clinically appropriate.

Administrative Responsibilities:

• Attends required staff meetings, departmental meetings, medical staff meetings, trainings, and educational activities as scheduled or assigned.
• Participates in program development, process improvement initiatives, and systems redesign projects to enhance service line performance and Veteran satisfaction.

You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Pay: Competitive salary, annual performance bonus, regular salary increases

Paid Time Off: 50-55 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year and possible 5 day paid absence for CME)

Contract: No Physician Employment Contract and no significant restriction on moonlighting

Work Schedule: Monday - Friday, 7:30am - 4:00pm
