Care Navigator, Social Worker

Description

As a Care Navigator you are a member of the dementia care and caregiver interdisciplinary team. The Care Navigator provides comprehensive care coordination services and advanced care planning in order to maintain a long-term care partnership for patients living with dementia or other diseases and their care partner(s). The Care Navigator will work collaboratively with the medical director and provide oversight. The Navigator will provide comprehensive and integrated care with the goals of improving the quality of life for patients living with dementia or other life-threatening diseases and their care partner(s). The Navigator is trained in dementia assessment and care planning, providing a standardized set of services and extensive dementia and Alzheimer's education.

Requirements

Minimum Qualifications:

• Must be a supportive team member, contribute to, and exemplify teamwork and collaborative team concept.
• RN, LPN, or Social Worker is preferred
• Must have excellent verbal and written communication skills
• Must be self-directed, detail-oriented, and organized, able to manage responsibilities under little supervision, with a strong work ethic.
• Demonstrate a positive attitude, self-awareness, and emotional maturity to promote helpful, respectful, and supportive communications.
• Familiarity with computer systems, including Microsoft Office, and demonstrated ability to use computer programs for training and communications.
• The knowledge base of dementia includes causes, symptoms, and management of dementia, and family support strategies. Understanding of Medicare and other long-term care insurance policies. Demonstrated understanding of terminal illness, death and dying, loss and grief.
• Demonstrated ability to apply analytical and problem-solving skills. Demonstrated ability to manage multiple tasks or projects effectively.
• Ability to work efficiently in a fast-paced environment with changing priorities, managing time well, performing assigned duties with attention to detail, accuracy, and follow-through.
