About This Role The Case Manager RN provides leadership in coordinating patient-centered care, developing discharge plans, and collaborating with multidisciplinary healthcare teams to ensure optimal transitions in care. Highlights Pay: $60,200 - $107,400/year Location: Bangor, ME Facility: Northern Light Health Brand/Division: Optum Schedule: Full Time, Day shift What You’ll Do Problem-solve and actively pursue resolution. Communicate directly with staff, physicians, patients, and families. Model leadership behavior through courtesy, respect, and efficiency. Coordinate patient care processes to achieve quality outcomes. Facilitate patient and family education to promote continuity of care. Review patient plans of care with the multidisciplinary team. Communicate with patient care staff for continuous assessment and planning. Function without direct supervision, organizing assignments for productivity. Adhere to name badge and dress code compliance. Requirements Education Associate’s degree in Nursing required. Experience 3+ years of experience in a hospital, acute care, or direct care setting required. Licenses & Certifications Current, unrestricted compact RN license in the state of residence required.
