RN Case Manager/Preceptor - Home Health

Connected Home Health is seeking experienced and caring Home Health Registered Nurse (RN) Case Managers with precepting experience to join our team. We offer flexible work schedules and the opportunity to provide life‑changing service to patients in their homes. Salary: commensurate with experience (starting at $53,000). Connected Home Health is a Medicare‑certified Home Health Agency serving the greater Portland, OR area for over 16 years. Essential Job Functions / Responsibilities Provides direct patient care as defined in Oregon’s Nurse Practice Act. Implements plan of care. Provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse. Qualifications Graduate of an accredited practical nurse or vocational nursing program. One year of nursing experience preferred. Currently licensed as a registered nurse in Oregon. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organization skills. Licensed driver with a vehicle insured in accordance with state requirements and in good working order. Possesses and maintains current CPR/BLS certification from AHA.
