If so, apply today!As a **Registered Nurse Case Manager,**you’ll contribute to our success in the following ways:* Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.* Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.* Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.* Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.* Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.**What is Required?*** Current State license as a Registered Nurse.* Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing* 1+ year of experience as a nurse in a clinical care setting.* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.* Experience in hospice or home health environment is *preferred***What's in it for you?*** Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance* 401(K) with employer match* Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost* Paid time off, paid holidays, family, and pet bereavement* Fleet vehicle program (restrictions apply) or mileage reimbursement.\\* Benefit eligibility can vary by position and shift status.***Equal Employment Opportunity**: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
