Key Responsibilities Conduct comprehensive psychosocial assessments through interviews, observations, and review of patient medical records to identify emotional, social, and environmental factors impacting recovery. Deliver counseling and emotional support to patients and their families, effectively addressing adjustment issues related to rehabilitation and helping them navigate their feelings during this transition. Collaborate with an interdisciplinary rehabilitation team to design and implement individualized treatment plans tailored to the specific psychosocial needs of each patient. Facilitate access to vital community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring patients have the tools they need for successful rehabilitation. Provide ongoing education and resources to patients and families regarding post-rehabilitation services and community options that may enhance their quality of life. Engage in discharge planning, assisting with the smooth transition of patients to home or other care settings while ensuring continuity of care. Patient Population / Client Focus Your work will significantly impact a diverse patient population encompassing individuals facing a wide range of physical and mental health challenges. The clients you serve may include those recovering from strokes, spinal cord injuries, orthopedic surgeries, neurological disorders, and other life-altering conditions. Your empathetic approach will help patients navigate their physical rehabilitation while addressing the emotional and social adjustments required for successful reintegration into their communities. Qualifications A Master's degree in Social Work from an accredited program is required. Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is essential. Compensation and Benefits The annual salary for this position ranges from $75,000 to $100,000, depending on experience and qualifications, along with the opportunity for a sign-on bonus.
