Responsibilities
- Perform initial and ongoing comprehensive assessments of the patient’s needs; conduct nursing assessments and update the plan of care as needed.
- Develop, implement, and coordinate the plan of care with input from the patient, family, and the care team; adjust as goals evolve.
- Educate patients and families on nursing care, patient rights, and care plans, providing written materials as appropriate.
- Document patient care, status, responses to treatment, and interventions in a timely, legible manner; maintain and update medication profiles.
- Collaborate with multiple disciplines, serving as a resource to patients and families to achieve mutually agreed-upon goals and safe, high-quality outcomes.

Requirements
- Active Registered Nurse license in the state of practice (California)
- Bachelor’s Degree in Nursing (BSN) preferred
- At least two years of recent clinical experience in the field
- Current CPR/BLS certification from an approved vendor (or AH policy equivalent)
- EPIC electronic health record experience required
- Valid Driver’s License and automobile insurance; must be at least 21 years of age; reliable vehicle
