Shift: 8:00am - 5:00pm

Job Summary

We're seeking a highly organized, detail-oriented, and customer-focused Rehabilitation Coordinator to support Michigan Rehabilitation Services (MRS) in Port Huron, MI.

The Rehabilitation Coordinator will assist customers throughout the rehabilitation services process by conducting thorough intakes, explaining the MRS six-step rehabilitation process, collecting required documentation, and providing employment-related assistance.

This position also involves maintaining accurate case records, documenting customer interactions, and assisting customers with job search activities and pre-employment workshops.

Essential Duties and Responsibilities

Customer Intake and Support

• Conduct thorough customer intakes and ensure all required information is collected and documented accurately.
• Explain the MRS six-step rehabilitation process, program requirements, and next steps to customers.
• Provide ongoing support and follow-up to assist customers throughout the rehabilitation services process.
• Communicate professionally with customers in person, by telephone, email, and through virtual platforms.
• Assist with scheduling appointments and coordinating customer services.

Documentation and Case Management

• Follow up with customers to obtain required medical records, psychological evaluations, employment paystubs, and student grades.
• Enter and update customer information in the electronic case management system.
• Maintain detailed case notes documenting services provided, customer interactions, and follow-up activities.
• Review customer records for accuracy, completeness, and outstanding documentation.
• Maintain confidentiality of sensitive customer information in accordance with established policies and procedures.

Employment and Job Search Assistance

• Assist customers with online job searches, employment applications, and identifying job opportunities.
• Provide basic guidance on navigating employment websites and online job search resources.
• Assist customers with participating in pre-employment workshops and job readiness activities.
• Encourage customer participation in employment preparation and rehabilitation services.

Administrative Support

• Maintain organized and accurate electronic customer records.
• Monitor outstanding documentation and conduct timely follow-ups.
• Assist with appointment scheduling, data entry, and other administrative responsibilities.
• Follow established MRS procedures, guidelines, and documentation requirements.
• Manage multiple priorities while maintaining accuracy and meeting deadlines.

Required Education:

Possession of a bachelor's degree with a major in one of the following areas:

• Rehabilitation Counseling
• Rehabilitation Services
• Secondary Education
• Special Education
• Business
• Human Resources
• Social Work
• Psychology
• Guidance and Counseling
• Occupational Therapy

Candidates with relevant experience in the following areas are encouraged to apply:

Preferred Qualifications

• Experience working in rehabilitation services, social services, workforce development, or a related environment.
• Familiarity with Michigan Rehabilitation Services (MRS) programs and processes.
• Experience using electronic case management systems.
• Experience assisting individuals with disabilities or barriers to employment.
• Previous experience providing virtual customer support.
