RN Case Manager: Care Coordination & Discharge Planning

HCF is seeking a Registered Nurse Case Manager to coordinate skilled care and communicate with the interdisciplinary team, residents and families. This role ensures adherence to physician orders and promotes quality outcomes at our Coudersport facility. Responsibilities include pre-admission assessments, admission orders, MDS coordination, QA involvement, and ongoing discharge planning.
