RN Case Manager: Discharge & Care Coordination

Trinity Health MI seeks a Registered Nurse Case Manager to lead discharge planning and coordinate care across the continuum to achieve clinical, financial, and utilization goals. You will assess patient goals, review care levels, and develop discharge plans with patients, families, and the interprofessional team, adjusting as needs evolve. This full-time, benefited role includes orientation 6–8 weeks, 32 hours per week, day shift, with opportunities for growth and CMSA certification support. #J-18808-Ljbffr Trinity Health MI
