Nurse Care Manager

Kaiser Permanente is seeking a Nurse Care Manager to coordinate and optimize patient care within our Healthcare & Medical Services department. In this role, you will perform comprehensive assessments, develop and manage individualized care plans, and coordinate services across providers, hospitals, and community resources. You will leverage your clinical expertise to support chronic disease management, safe care transitions, and utilization management while ensuring high-quality, patient-centered outcomes. Using electronic health records and standardized care pathways, you will collaborate closely with physicians and interdisciplinary teams to close care gaps, educate patients and families, and promote preventive care.
