Licensed Mental Health Counselor

Position Overview The Fusion Recovery Center Clinician (LMHC) plays a critical role in providing comprehensive counseling services for individuals experiencing substance use and co-occurring mental health disorders. This position offers the opportunity to work in an integrated outpatient setting, delivering individual and group therapy using evidence-based practices in compliance with OASAS regulations. As a Licensed Mental Health Counselor and Qualified Health Professional (QHP), the Clinician is responsible for conducting diagnostic assessments, developing treatment plans, and delivering clinically appropriate interventions. The Clinician will complete psychosocial intake assessments and collaborate closely with the Program Director, Clinical Supervisor, support staff, medical team, and other clinicians to ensure coordinated, person-centered care. Key Responsibilities Conduct comprehensive biopsychosocial and diagnostic assessments in accordance with OASAS standards, including level-of-care determinations (e.g., LOCADTR). Maintain a personal caseload and provide individual, group, and family psychotherapy. Utilize evidence-based practices (e.g., Motivational Interviewing, Cognitive Behavioral Therapy, trauma-informed care) to address substance use and mental health disorders. Coordinate client intakes, including admission documentation (e.g., PAS-44) and collaboration with medical providers for integrated care, including medication-assisted treatment (MAT) when applicable. Develop, implement, and regularly update individualized treatment plans in accordance with OASAS timelines and medical necessity requirements. Maintain accurate, timely, and compliant clinical documentation in the electronic medical record (EMR), ensuring alignment with Medicaid APG billing standards. Coordinate care with external stakeholders, including CPS, probation, and other community agencies, while adhering to 42 CFR Part 2 and HIPAA confidentiality regulations. Orient clients to program policies, patient rights, and treatment expectations; facilitate monthly orientation sessions for new admissions. Monitor and reconcile census data and ensure timely submission of required OASAS data reporting forms (e.g., PAS-44/45). Participate in interdisciplinary team meetings, case conferences, and clinical supervision. Provide psychoeducation on substance use disorders, mental health conditions, relapse prevention, and community resources. Conduct and document toxicology screenings as clinically indicated. Support recovery efforts within an integrated care model, including collaboration with medical staff providing MAT. Engage in ongoing professional development and maintain compliance with all OASAS and NYS licensure requirements. Education and Qualifications Master’s degree in Mental Health Counseling or a related field from an accredited institution. Active and unrestricted Licensed Mental Health Counselor (LMHC) license in New York State. Must meet criteria as a Qualified Health Professional (QHP) under OASAS regulations. Experience working with substance use disorders and co-occurring populations preferred. Knowledge of OASAS regulations, Medicaid APG billing, and integrated outpatient treatment models strongly preferred.
