Registered Nurse Full Time Professional San Jose, CA, US Salary Range: $45.00 To $50.68 Hourly Registered Nurse Seeking Private Duty Registered Nurses to provide in-home nursing care with a focus on high standards of care, integrity, and reliability. Hallway Healthcare provides in-home nursing care to retired Nuclear and Federal Workers who have benefits through EEOICPA or FECA. We also serve Federal Workers such as US Postal Workers. Current California RN/LVN License Current BLS Certificate Schedule/Shifts: Location: San Jose, CA Available Shifts: Monday-Sunday, 8 hours a day Full-time, Part-time, Per diem available Pay rates: $45.00-$50.68/hr Daily rate: $630-$709/day We offer competitive shift differentials, including an additional $5/hour for 8-hour shifts and $10/hour for shifts under 8 hours. For shifts of 8 hours or more, overtime (time-and-a-half) is paid for all hours worked beyond 8 in a day. Other locations and shifts available! Responsibilities / Essential Functions: Demonstrate efficient teamwork with the staff. Demonstrate organizational and time management skills. Support quality improvement practices. Perform nursing procedures according to agency policy and procedures. Work under the direction of an RN Case Manager and Director of Nursing. Monitor reactions and patient progress using observation, assessment, and evaluation skills. Educate patients and family members according to disease process, medications, plan of care, and home care procedures. Report to physician and RN Case Manager of adverse findings. Follow state regulations. Complete clinical notes for each visit and incorporate into the record on a daily basis. Coordinate and monitor patient care and services. Comply with HIPAA regulations. Follow infection control policy. Light housekeeping and meal preparation. Transport patients to doctor appointments, grocery stores, or pharmacies. Document skilled visits accurately and in accordance with established guidelines. Documentation must be completed by the end of each shift; notify Case Manager if unable to complete. Maintain patient records according to policy and procedures. Participate in in-services, workshops, seminars, and self-study courses.
