Following each patient's plan of care, the LPN provides direct care and documents each visit using the Netsmart EMR system; reporting any changes in the patient's condition to the RN Case Manager. LPN can choose from (2) work schedules: 8am to 4 pm OR 4 pm to 12 midnight Qualifications: Graduate from an accredited Licensed Practical Nurse Program Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience Good PC skills a must Drivers license and care required for all visits except in Manhattan Jobot is an Equal Opportunity Employer.
