101 Riverside Hospital, Inc.

Overview Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN may work in a variety of settings in the Medical/Surgical Complex and roles within Riverside Health System. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial, and cultural diversity. Responsible for supervising and delegating non‑professional personnel working directly with her/him to ensure optimum patient care. Must demonstrate competence in dealing with age‑related needs of the patient populations served in the Medical/Surgical units/departments. Works under the direction of leadership and a multi‑disciplinary team to provide a comprehensive range of services or clinical care in accordance with standards of excellence established by Riverside Health System and facility goals and strategic plans. Responsibilities Assess, plan, implement, and evaluate the total nursing care of assigned patients on a caseload on an intermittent visiting basis. Develop initial, ongoing, and comprehensive assessments of client needs, including the impact of terminal diagnoses on physical, functional, psychological, and environmental needs. Assess patient risk for pathological grief, cultural and spiritual implications, and communication patterns. Evaluate caregiver ability to meet patient needs upon admission and throughout care. Determine scope and frequency of services needed based on acuity and patient/family needs. Accurately complete all required clinical documentation per RHS standards, including patient notes, verbal orders, certifications, and recertifications, within monthly deadlines. Apply admission and re‑certification criteria to hospice care to determine appropriate levels of care and patient eligibility. Initiate and revise individualized plans of care and provide symptom relief and specialized nursing treatments related to palliative and end‑of‑life care. Consult, counsel, and educate patients, families, and staff on nursing needs, disease processes, self‑care techniques, end‑of‑life care, and ethical concerns. Use a case‑management approach and refer patients to additional services as needed, coordinating all patient/family services and prioritizing needs with interdisciplinary team members. Continuously evaluate patient/family response to care and inform physicians and the interdisciplinary team of changes in patient needs and plan of care. Initiate preventive and rehabilitative nursing procedures to promote physiological and psychosocial outcomes and stabilize the patient. Prepare clinical and progress notes demonstrating progress toward established goals. Supervise LPN/LVN staff and Home Health Aides in providing services according to regulatory guidelines. Provide specialized hospice training to staff, family members, and informal caregivers to ensure adequate care. Participate positively in training and support of all team members, volunteers, and students as needed. Delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient needs are met. Recognize and respond to emergency situations. E‑valuate own needs for support and utilize identified systems and resources to meet them. Qualifications Education : Registered Nurse (Required); Bachelor’s Degree in Nursing (Preferred). Experience : Hospice experience (Preferred). Licenses and Certifications : Registered Nurse (RN) – Virginia Department of Health Professions (VDHP) – Required. CPR/BLS Certification : American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 days – Required. #J-18808-Ljbffr
