Social Worker - LMSW Profession: Social Worker Specialty: Social Work Duration: 17 weeks Shift: 8H Days Hours per Shift: 37.5 hours per week Experience: Minimum 2 years License: NY LMSW Required Certifications: BLS Certification Must-Have: Flu Vaccine Required - No Exemptions or Exceptions Description:The Licensed Social Worker assesses the patient's psychosocial problems and needs at admission and ongoing thereafter.In collaboration with the patient, family, significant others, and members of the treatment team, a treatment and aftercare plan is developed based on the identified psychosocial problems.The Licensed Social Worker acts as the primary therapist for assigned patients.Assessments of the patient's psychosocial problems and needs occur within the required time frame after admission and continue thereafter.Collaboration with the patient, family/significant others, and treatment team members takes place to carry out the treatment and aftercare plan.The social worker evaluates the patient’s responses to planned interventions and makes decisions regarding the continuation or revision of the treatment plan.Collaboration with physicians and other health care team members furthers understanding of significant social and emotional factors underlying the patient’s mental health issues.The social worker is responsible for discharge planning for assigned patients and arranges appropriate discharge plans in collaboration with the health care team.Advocacy for patients in their relationships with outside agencies is an essential duty.All phases of the treatment process are documented in the medical record within appropriate timeframes.Accountability for clinical competency, professional growth and development, and ethical practice is maintained.Participation in special projects and meetings in the community is expected as requested or according to professional interest. #J-18808-Ljbffr
