We're looking for a Psychiatric Provider to join our Safe Options Support (SOS) teams on a part-time basis, delivering psychiatric care to unsheltered individuals experiencing serious mental illness and co-occurring substance use disorders. Through field visits and telehealth, you'll meet members where they are - providing evaluations, medication management, and harm-reduction-oriented care, while lending clinical leadership to a dedicated multidisciplinary team. Candidate must be willing to travel throughout Onondaga & Oswego Counties on a regular basis.

Grade 313: This is a part-time position, 8 hours per week, working from home, but does require travel throughout Onondaga & Oswego Counties. The minimum and maximum annual salary that Elm & Oak Health believes in good faith to be accurate for this position at the time of this posting are $75/hr - $90/hr. The Psychiatric Provider is an integrated member of the Safe Options Support team, delivering comprehensive psychiatric care to unsheltered individuals experiencing serious mental illness, substance use disorders, and/or co-occurring conditions. The role encompasses direct clinical care, care coordination with outside providers, clinical leadership, and staff education, consistent with NYS OMH SOS program guidelines. Telehealth and field visits may be used flexibly to improve participant access. Travel days for case conferencing and staff education are included within the scope of the role.

Caseload Performance Expectations (OMH)

Milestone - Minimum Unique Individuals on Caseload

Within 6 months of hire - 4 unique individuals

End of first year (12 months) - 6 unique individuals

Annually thereafter - 14 unique individuals per year

Essential Job Duties/Functions

40% - Direct Psychiatric Care

Performs diagnostic evaluations, provides psychotherapy, and implements psychiatric medication regimens for SOS participants when clinically indicated.

Utilizes flexible modalities including field visits, engagement encounters, and telepsychiatry to improve access.

Implements long-acting injectable medications, Medication Assisted Therapy (MAT), clozapine therapy, and medication delivery support as clinically appropriate.

Apply evidence-based, harm-reduction-oriented prescribing practices, including once-daily dosing, short prescription intervals, and long-acting injectables to support adherence.

30% - Care Consultation & Coordination

Consults, advises, and coordinates with non-SOS psychiatric providers to clarify treatment roles, update diagnoses, and advocate for effective treatment plans.

Collaborate with the SOS team to communicate with hospitals and crisis services when indicated.

Coordinates care transitions between practitioners.

Reviews PSYCKES and other clinical data to inform treatment decisions and participant updates.

20% - Clinical Leadership & Team Education

Provides medical leadership for the SOS team.

Collaborates with the team to develop evidence-based clinical treatment plans and identifies staff clinical educational needs.

Delivers clinical trainings and provides input on case management support based on participant needs.

Reviews quality of team-provided care and collaborates with the Team Lead to continuously improve care quality.

Co-leads SOS Clinical Team Meetings (minimum every two months) and SOS Clinical Leadership Meetings (minimum every two months) with the Team Lead and Clinical Liaison.

10% - Scheduling, Documentation & Administrative

Maintains the psychiatric treatment schedule in collaboration with the Clinical Liaison, with appropriate time blocks for evaluations and follow-up visits.

Participates in sign-in and sign-out with the Clinical Liaison at the beginning and end of each shift to review the clinical schedule, new referral materials, and caseload updates.

Documents all treatment encounters, medication changes, and recommendations.

Adheres to SOS guidelines and key performance indicators as specified by OMH street psychiatry guidance.

Total must equal 100% - essential functions should be completed at least 10% of the time

• Demonstrate cultural awareness, cultural knowledge, and cultural skills in all aspects of job role.
• Attend and participate in team meetings and organizational/departmental trainings.
• Performs other duties as assigned.

Position Limitations:

• Cannot transport active Elm & Oak Health members at any time.
• Cannot perform hands on care.

• Active, unrestricted NYS license as a Nurse Practitioner (NP), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO)
• Board certification or board eligibility in Psychiatry (or Psychiatric-Mental Health Nurse Practitioner certification)
• Valid DEA registration with ability to prescribe controlled substances in NYS
• Experience providing psychiatric care to individuals with serious mental illness, substance use disorders, and/or co-occurring conditions
• Familiarity with PSYCKES and electronic medical record (EMR) documentation
• Ability to work in community-based, field, and telehealth settings
• Punctuality and reliable attendance are essential to this position
• Knowledge of homeless resources, shelter systems and transportation systems.

• Knowledge of counseling principles and methods for mental illness and substance use disorders.
• Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff.
• Demonstrates ability to respect individual/family diversity and maintain confidentiality.
• Demonstrates ability to work as a team member.
• Knowledge of and ability to work collaboratively with providers and county/community health and human services.
• Ability to demonstrate excellent communication skills both oral and written as well as strong interpersonal skills.
• Proven ability to work independently and to manage time appropriately.
• Strong organizational skills.
• Computer literate. Must be able to pass computer documentation competency testing for all software platforms used within the program. Candidates will need a NYS driver's license and to own or have access to reliable transportation that enables them to fulfill travel requirements of the job including but not limited to, daily visits to members' homes.

Preferred Qualifications

• Experience with street medicine, assertive community treatment (ACT), or other outreach-based psychiatric care models
• Experience administering long-acting injectable antipsychotics and/or Medication Assisted Treatment (MAT), including buprenorphine and extended-release naltrexone
• Familiarity with clozapine therapy protocols and monitoring requirements
• Experience working with individuals experiencing homelessness, housing instability, or involvement in the criminal justice system
• Training in trauma-informed care, motivational interviewing, and/or de-escalation techniques
• Bilingual or multilingual abilities a plus
