The Restorative Registered Nurse (Nursing Home) Opportunity:

The Restorative Nurse is responsible for coordinating, implementing, and monitoring restorative nursing programs designed to maintain and improve residents’ functional abilities, independence, mobility, and quality of life. The Restorative Nurse is an active member of the Interdisciplinary Team (IDT) collaborating with nursing staff, therapy services, providers, residents, and families to develop individualized restorative approaches based on each resident’s needs, goals, and care plan.

The Restorative Nurse provides clinical oversight of restorative nursing activities, ensures interventions are carried out consistently and appropriately, monitors residents’ progress and changes in condition, and communicates recommendations to the interdisciplinary team. This position also provides education and guidance to nursing staff regarding restorative techniques, promotes resident-centered care, and ensures accurate documentation and compliance with applicable regulatory and facility standards.

What You’ll Get:

• The starting pay range for this role is $40.02 - $43.58 and shift differential of $1.50 for 2:00pm – 10:00pm and $2.50 for 10:00pm-6:00am, based on education and experience.
• The hours for this role are 8:00am – 4:30pm on weekdays.
• This job is in-person and a full time 0.8 FTE.

What You Bring:

Required:

• Current Registered Nurse (RN) license in the State of Wisconsin.
• Current BLS (CPR) certification.
• More than 3 years of dementia care, skilled nursing or long-term care.
• Experience utilizing electronic health record (EHR) systems.
• Demonstrated experience coaching employees and supporting clinical operations.

Preferred:

• Previous experience coordinating or overseeing a restorative nursing program.
• Familiarity with the MDS (minimum data set) process.
• Experience collaborating with an Interdisciplinary Team (IDT) to support resident-centered care and functional outcomes.

What a Restorative Registered Nurse does at NCHC:

Clinical Leadership and Resident Care

• Provide clinical oversight and coordination of the facility’s restorative nursing program to promote residents’ highest practicable level of function, independence, and quality of life.
• Assess residents’ functional abilities and needs and develop, implement, and monitor individualized restorative nursing interventions in collaboration with the interdisciplinary team.
• Participate as an active member of the Interdisciplinary Team (IDT), contributing to care planning, resident progress, changes in condition, and recommendations for restorative services.
• Monitor residents’ progress and response to restorative interventions and make appropriate recommendations for modification, continuation, or discontinuation of services.
• Collaborate with nursing, therapy, providers, and other disciplines to promote continuity of care and support resident-centered goals.
• Identify changes in residents’ functional status and communicate concerns or recommendations to the appropriate members of the care team.
• Ensure restorative programs are accurately reflected in the resident’s plan of care and are implemented consistently.
• Maintain appropriate clinical documentation related to assessments, interventions, resident progress, and outcomes.

Regulatory Compliance and Quality Improvement

• Ensure restorative nursing practices are consistent with applicable federal and state regulations, professional standards, and facility policies and procedures.
• Monitor restorative documentation and program implementation for accuracy, completeness, and compliance.
• Participate in audits, quality assurance activities, and performance improvement initiatives related to restorative nursing and resident outcomes.
• Identify trends, gaps, and opportunities for improvement and collaborate with leadership and the IDT to develop and implement corrective actions.
• Assist with preparation for regulatory surveys and respond to identified areas of concern related to restorative nursing services.
• Monitor resident outcomes and program effectiveness and utilize data to support ongoing quality improvement.

Reimbursement and Capturing Restorative Services

• Ensure restorative services and interventions are appropriately identified, documented, and captured to support accurate reimbursement and reflect the care provided to residents.
• Review restorative documentation and records for accuracy, consistency, and alignment with resident care plans and assessed needs.
• Collaborate with MDS, nursing, therapy, and other appropriate team members to ensure restorative services are communicated and accurately reflected in the resident’s clinical record and applicable assessment processes.
• Identify missed or incomplete documentation that may impact accurate reimbursement and work with staff to address identified gaps.
• Maintain knowledge of current restorative nursing documentation and reimbursement requirements applicable to the facility.

Coaching and Staff Development

• Provide ongoing education, coaching, and competency support to nursing staff regarding restorative nursing techniques, interventions, documentation, and resident-centered approaches.
• Coach staff on proper implementation of restorative programs and reinforce consistency in care delivery.
• Observe staff performance and provide constructive feedback to promote adherence to restorative plans and facility expectations.
• Assist with orientation and competency validation for staff involved in restorative nursing programs.
• Serve as a clinical resource and mentor to nursing staff regarding functional maintenance, mobility, activities of daily living, and restorative interventions.
• Promote a culture of accountability, teamwork, and continuous improvement related to resident function and quality of care.

MDS Support and Backup

• Provide backup support for MDS-related functions as needed.
• Assist with gathering and validating clinical information necessary for accurate MDS completion.
• Collaborate with the MDS team and interdisciplinary team to support timely and accurate documentation.
• Assist with identifying changes in resident status that may impact MDS assessments and care planning.
• Maintain familiarity with MDS requirements and applicable documentation standards.

Staff Leadership and Supervision

• Provide clinical leadership and guidance to nursing staff related to restorative nursing practices and resident functional outcomes.
• Serve as a clinical resource to nursing staff and promote consistent implementation of restorative programs.
• Provide constructive feedback and education to support staff competency and quality of care.
• Participate in the nursing on-call rotation and provide clinical guidance and support to staff as needed.
• Escalate significant clinical concerns, performance concerns, or resident care issues to the appropriate leader.
• Collaborate with nursing leadership to support staff accountability, quality improvement, and resident-centered care.

Working Environment and Physical Requirements

• Frequent standing, walking, bending, reaching, stooping, twisting, pushing, pulling, and lifting during the performance of duties.
• Ability to lift and move up to 25 pounds regularly and greater weights occasionally with or without assistance.
• Frequent use of computers, electronic medical records, communication systems, and office equipment.
• Requires visual acuity, hearing, verbal communication, and written communication sufficient to assess residents, review documentation, and perform job responsibilities.
• Work is performed in both office and resident care environments.
• Regular exposure to residents with complex medical, cognitive, behavioral, or psychiatric needs.
• Frequent exposure to communicable diseases, bodily fluids, bloodborne pathogens, and other potentially infectious materials.
• May be exposed to hazardous chemicals, cleaning agents, medical equipment, and biological hazards.
• Occasional exposure to agitated, disruptive, hostile, or combative individuals.
• May be required to wear personal protective equipment, including respirators such as N95 masks.
• Minimal travel may be required for meetings, training, or organizational activities.
• Participation in an on-call rotation and availability outside normal business hours may be required.
