Physical Therapist

We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will provide exceptional patient care in various settings, including hospitals, outpatient clinics, and home health environments. This role involves assessing patient needs, developing personalized treatment plans, and guiding patients through rehabilitation exercises to improve mobility and quality of life. Duties Conduct comprehensive evaluations of patients to determine their physical therapy needs. Develop and implement individualized treatment plans based on patient assessments. Provide therapeutic interventions for patients in various settings, including inpatient, outpatient, and home health environments. Collaborate with multidisciplinary teams to ensure comprehensive patient care and effective discharge planning. Educate patients and their families on therapeutic exercises, injury prevention, and overall health maintenance. Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes. Skills Strong knowledge of physical therapy practices in various settings such as orthopedics, geriatrics, acute care, and home community care. Excellent communication skills for effective interaction with patients, families, and healthcare team members. Ability to work collaboratively within a multidisciplinary team environment. Experience with discharge planning processes to ensure smooth transitions for patients post-treatment. Familiarity with both inpatient and outpatient rehabilitation protocols. Compassionate approach towards patient care with a focus on improving quality of life through rehabilitation.
