Traveling Physical Therapist Opportunity

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Physical Therapist license in Indiana required to start.
• HEP B required.
• TB required.
• Flu required.
• Tdap required.
• Criminal Background required to start.
• OIG Search required to start.
• Statement of Good Health required to start.
• OSHA Packet required to start.
• Verification of Professional Education required to start.
• I-9 Form required to start.
• GSA Search required to start.
• UDS - 9 Panel (Marijuana Not Included) required to start.
• Contract Therapist Compliance Attestation required to start.

Assignment Details

• Monday and Wednesday, 8:00 AM–5:00 PM, with the last patient at 4:00 PM.
• Tuesday and Thursday, 10:00 AM–7:00 PM, with the last patient at 6:00 PM.
• Friday, 7:00 AM–4:00 PM, with the last patient at 3:00 PM.
• One-hour lunches are included.
• Documentation system: NextGen.
• Floating to another clinic may be considered if staffing is secured.
• Patient population includes outpatient orthopedic patients.
