Diabetes Educator (8a-5p) Pediatric Endocrinology - Johnson City, TN

Job Description: Summary: The Diabetes Educator is responsible for direct patient care in the provision of professional diabetes care management of patients in the Diabetes disease management program. The Educator will have working knowledge of diabetes care management including development and utilization of education and assessment tools and guides. The Educator will demonstrate skill in techniques of good patient care. In addition, the Educator will provide monthly or periodic follow-up visits and/or calls to participants. Additionally, the Educator will maintain confidentiality of participant at all times while documenting appropriately. Additionally, the Educator will interact with Primary Care Physician and Pharmacist as needed to provide a seamless transition of care and information. This individual will utilize health coaching strategies and motivational interviewing techniques when interacting with diabetes care management participants. The Educator is responsible and accountable for the care of assigned patients. The Educator is expected to practice autonomously within the scope of professional standards, the rules and regulations established by the State of Tennessee Division of Health Related Boards, and knowledge of the policies and procedures within Ballad Health. It is vital to have a working knowledge of the diabetes care management process and demonstrate skills in developing and guiding collaborative collegial practice that delivers safe competent care. The Educator will provide resources, education, training, for peers/other providers, and will assist with development of program teaching/training tools and guides.

Requirements: An associate's degree from an accredited nursing program (bachelors or masters degree preferred) or Certified Dietetic Registration (CDR) certification and a bachelor's or master's degree in nutrition required or other related training. Minimum three to five years clinical experience (with emphasis on diabetes management) in a hospital, home health, or occupational health setting with substantial public contact preferred. Experience in case management and/or disease management. Must possess and maintain current certification in cardiopulmonary resuscitation. The Diabetes Educator must have dependable transportation with current personal collision and liability insurance on any vehicle driven.

Licenses and Certifications: He/she must also maintain a current driver's license in the appropriate state. Valid and active RN licensure in appropriate state or Certified Dietetic Registration CPR required Valid and active Diabetes Educator certification preferred upon hire or must obtain within 2 years
