Qualifications To excel in this role, you'll need: A current PT / Physical Therapy license in the State of Minnesota. Experience in a home care setting, including proficiency in OASIS documentation. A valid Driver's License and reliable transportation. Exceptional communication and interpersonal skills. The ability to work autonomously as well as collaboratively within a team. Proficiency in Microsoft Office programs. Availability to travel to patient homes in the local area.
