Value Based Care Leader for Primary Care Medical Group office in Germantown. Lead two Advance Practice Providers, plus collaborate with Operational and Regional leaders driving quality outcomes to support our high-risk patients.

We are seeking Primary Care Physicians who desire the opportunity and experience of delivering quality and compassionate healthcare - within proven care models to elderly individuals and those with complex medical issues, who are the forefront of everything we do.
As a Primary Care Physician Leader, you will treat the complex chronic conditions of our geriatric patients in their home. A Medical Assistant will support vitals, labs, EKGs in preparation of the visit. You will have clinical oversight of Advance Practice Provider, NPs.

- Experience in Geriatrics or complex patients preferred
- Active/unrestricted medical license and DEA
- Board Certified or Board Eligible
