Conduct supervisory patient visits and quality assurance visits. Assess patient conditions and identify age-specific and diagnosis-related care needs. Review, monitor, and maintain accurate, timely, and compliant clinical documentation. Investigate and resolve patient, family, and employee concerns professionally and promptly. Collaborate with physicians, patients, caregivers, and interdisciplinary healthcare teams to coordinate care. Provide clinical guidance, education, and support to nursing staff and caregivers. Participate in quality improvement initiatives and ensure compliance with company policies and regulatory standards. Utilize critical thinking and sound clinical judgment to resolve issues and improve patient outcomes. Qualifications Required: Active and unrestricted Registered Nurse (RN) license in the State of Indiana Minimum of two (2) years of recent home health nursing experience Valid driver's license and reliable transportation Strong assessment, documentation, and clinical decision-making skills Experience with electronic medical records (EMR/EHR) Excellent communication, organization, and leadership abilities Preferred: Experience with Medicare home health regulations OASIS documentation experience Case management or clinical supervisory experience Bilingual candidates are encouraged to apply Patient Population Adult Geriatric Schedule Every other weekend- Saturday & Sunday Flexible scheduling Local travel required Apply Today!
