MDS Coordinator Nurse
Join Evergreen Healthcare Group at Rawlins Rehabilitation & Wellness and turn your nursing job into a fulfilling career! We’re seeking an MDS Coordinator Nurse to lead and manage the RAI/MDS process and support resident‑centered care as part of a collaborative IDT team.
What You’ll DoCoordinate timely MDS assessments and ARDs
Complete CAAs and care plan items
Ensure documentation supports accurate MDS coding
Participate in Medicare utilization management
Required: LPN or RN licensure + EMR proficiency
Preferred: Long‑term care experience, RAI/MDS background, PCC experience
