MSW Social Worker (LICSW) - Home Care - Full-Time

Schedule:  Full-Time (0.8FTE), 4 days/week 8am - 4:30pm, no weekends. May be open to 3 days/week for the right candidate.

Location: This position primarily serves the Stillwater, MN area/Washington County and St. Croix County in WI .

Pay rate:  Up to $70/hour Depending on Experience!

*This is a temporary assignment that is expected to last around 3-4 months, potentially longer.

What we ask of you:

• Complete home care visits and phone visits with patients, usually around 3 visits per day.
• Provide services to help meeting the patient/family's social, emotional, economic and environmental needs in relation to the impact of chronic, acute or terminal illness.
• Complete Advanced Directives.
• Be a resource for patients and help with county assistance, grief support, mental health support, etc.
• Complete conference calls with family members and involved team members of the interdisciplinary team.
• Provide patients/families with psychosocial support needed to cope with chronic, acute or terminal illness.
• Complete accurate and timely documentation of services provided. 

 

Required Skills/Qualifications:

• An active  LICSW license in the state of MN or WI, and willing to obtain both .
• An a MSW in Social Work from a CSWE accredited program.
• At least  one year of previous experience working as a LICSW, preferably in a home care setting. 
• Must have reliable transportation for client visits.
• Strong psychosocial assessment skills and impementation of interventions.
• Ability to function independently and within an interdisciplinary team.
• Computer experience including documentation in an electronic medical record, and effective written/verbal communication skills.
