Work Environment\n

This position is based remotely, allowing for flexibility while serving our clients at the Hospice Center. Your work hours will typically be Monday through Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend work based on client needs. You should be prepared for on-call availability to ensure our clients receive timely support during urgent situations.

Key Responsibilities\n

• Perform in-depth assessments to understand the psychosocial needs of seniors, focusing on emotional, mental, and social factors.

• Develop tailored care plans in collaboration with clients and their families, which incorporate their specific preferences and goals.

• Provide counseling and emotional support, assisting clients in navigating challenges such as life transitions and stressors.

• Advocate on behalf of clients to ensure access to necessary community resources, services, and support systems.

• Engage in interdisciplinary collaboration with nurses, caregivers, and other healthcare professionals to coordinate and enhance client care.

• Conduct regular client visits to monitor well-being, assess living conditions, and address any health or safety concerns.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker, you will primarily work with seniors and their families at our Hospice Center. This role requires a deep understanding of the unique emotional and psychological issues that arise in the context of aging, terminal illnesses, and end-of-life care. • A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience and qualifications.
