Purpose and Scope Provides psychosocial services to patients treated by the facility, including in‑center and home dialysis patients (if applicable), utilizing Social Work Theory of Human Behavior and acceptance methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation, and improved quality of life for patients. In collaboration with the interdisciplinary team, informs, educates, and supports staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families to ensure comprehensive quality care. Principal Duties and Responsibilities Patient assessment, care planning, and counseling: assess psychosocial status, strengths, and areas of need; develop interventions; provide monitoring and adjustments related to dialysis and psychosocial status. Support counseling services: deliver supportive counseling within scope of training and license; provide educational and goal‑directed counseling to patients seeking transplant; assist with referrals to community resources and vocational rehabilitation. Advance directive and DNR facilitation: assess patient awareness, assist with documentation, and discuss wishes with the healthcare team and family. Quality and quality improvement: provide psychosocial support to address non‑adherence, report on quality indicators, participate in monthly quality review meetings, and contribute to QAI activities. Patient education: assess knowledge of kidney disease; tailor education to learning style and barriers; collaborate on transplant referral processes and wait‑list management. Patient admission and continuity of care: review rights and responsibilities, grievance procedures, and important numbers; support clinic scheduling for adherence and adjustment. Insurance and financial assistance liaison: collaborate with Insurance Coordinators to address coverage issues, provide payment education, and refer patients to billing solutions. Staff support: assist with hiring processes, maintain resource lists, provide training on psychosocial topics, participate in weekly huddles, and discuss urgent patient issues. Adhere to caseload guidelines based on state regulations and perform related duties as assigned. Physical Demands and Working Conditions Travel may be required for multiple facilities or home visits if applicable. Education and Required Credentials Master in Social Work with a current state license in the state of employment. Must meet the applicable scope of practice board and licensure requirements. Experience and Skills 0–2 years of related experience.
