Key Responsibilities

• Conduct thorough health assessments, physical examinations, and screenings for patients of all ages in the community.
• Diagnose and manage acute and chronic medical conditions, develop treatment plans, and provide education on preventive care and health promotion practices.
• Work closely with community organizations and stakeholders to design and execute health education programs that address prevalent health concerns.
• Advocate for patients’ rights and ensure access to essential healthcare services while providing culturally sensitive care.
• Coordinate patient referrals and follow-up care, ensuring continuity in healthcare delivery.
• Maintain accurate and detailed medical records, adhering to all regulatory standards and privacy laws.
• Participate in community outreach events and health fairs, providing essential health screenings and raising awareness of health issues.
• Keep abreast of the latest trends in community health, evidence-based practices, and relevant healthcare policies.

Patient Population / Client Focus

Your primary focus will be on providing care to a diverse range of patients, including families and individuals from underserved communities. Qualifications

• Current Nurse Practitioner certification issued by a recognized national certifying body.
• Active and unrestricted nursing license in the state of New York.
• A Master’s degree in Nursing from an accredited academic program.
• Minimum of 3 years of clinical experience as a Nurse Practitioner, with a preference for experience in community health or public health settings.
• Exceptional interpersonal and communication skills, with experience working effectively with diverse populations.
• A commitment to health equity, social justice, and advocating for underrepresented communities.

Preferred Experience

• Experience in health education, preventive care, and chronic disease management is highly desirable.

Compensation and Benefits

We offer a competitive annual salary range of $180,000 to $200,000, commensurate with your experience and qualifications. Our comprehensive benefits package includes:
