We are seeking a dedicated and clinically skilled Licensed Social Worker to join our interdisciplinary team at Kadima Rehabilitation & Nursing at Mt. Lebanon. As a valued member of our team, you will play a crucial role in providing high-quality care and support to our residents and their families.

About the Role:

• As a Licensed Social Worker , you will be responsible for assessing the social and emotional needs of our residents and developing individualized care plans to address their unique needs.

• You will work closely with our interdisciplinary team, including physicians, nurses, and therapists, to ensure comprehensive care and support for our residents.

• You will provide counseling, support, and education to our residents and their families to promote emotional well-being, independence, and overall quality of life.

• You will maintain accurate and timely records of resident assessments, care plans, and progress notes.

Responsibilities:

• Conduct initial assessments of new residents to identify social and emotional needs.

• Develop and implement individualized care plans to address resident needs.

• Provide counseling, support, and education to residents and their families.

• Participate in interdisciplinary team meetings to discuss resident care and progress.

• Maintain accurate and timely records of resident assessments, care plans, and progress notes.

• Identify and report potential social and emotional concerns or issues affecting resident care.
