Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members. Ability to work independently with minimal supervision. Demonstrated ability to prioritize in a fast-paced environment. Experience and competency working with people from diverse backgrounds and cultures. Description: The Physical Therapist Assistant, under the direction of a physical therapist, is responsible for assisting with performing tests and providing restorative and rehabilitative Physical Therapy services to participants. Services are provided in a center and at home to maximize participant independence and safety and develop or restore function. The Physical Therapist Assistant will work with other members of the rehabilitation team to implement the plan of care established by the physical therapist. This role includes administering treatment programs as established by the Physical Therapist. Assist the Physical Therapist in obtaining vital signs, range of motion, and strength testing. Coordinate with other members of the rehabilitation team to implement activities and programs based on the participant’s individual treatment plans. Handle scheduling of participants' treatment and clinical visits. Document Physical Therapy interventions in accordance with the established care plans, including the use of devices and techniques to promote various factors affecting independence in performing daily living activities. Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment. Track and manage maintenance for equipment. Remain alert to input from team members, participants, and caregivers, and update the Physical Therapist promptly regarding any changes in participants' condition or functional status. Participate under the supervision of the Physical Therapist in end-of-life care coordination and support. Be legally authorized to practice in the position location and act within the scope of authority to practice. Complete all required documentation in a timely and accurate manner. Protect privacy and maintain confidentiality of all procedures and information about team members, participants, and families. Follow company policies and procedures and participate in any required quality improvement activities, staff training, and meetings. Communicate regularly with the supervisor and team regarding workload and priorities. Complete all mandated trainings and education on time. Complete all mandated physical health screenings as needed. Exercise flexibility in performing assignments as business needs evolve. Perform other duties as assigned.
