Primary Responsibilities:

• Conduct comprehensive physical assessments to determine patient strengths, limitations, and pain concerns.
• Develop and implement personalized treatment plans to help patients improve mobility and regain independence.
• Provide evidence-based interventions to enhance coordination, strength, and balance.
• Collaborate closely with physicians, nurses, occupational and speech therapists, as well as families to ensure cohesive care.
• Track and document patient progress, adapting therapy plans as needed and maintaining compliance with regulatory standards.
• Educate patients and caregivers on safe practices, home exercise routines, and equipment.
• Supervise PTAs and aides to support effective rehabilitation.
• Participate in regular care conferences and contribute to ongoing improvement in patient outcomes.

Qualifications:

• Graduate of an accredited Physical Therapy program.
• Current state license to practice as a Physical Therapist.
• CPR certification (preferred or depending on facility requirements).
• Experience in a SNF or with geriatric rehab preferred.
• Outstanding communication and teamwork skills.
