Hospital Pharmacist Jobs – Inpatient/Outpatient Travel Contract in San Francisco, CA

Hospital Pharmacist Job Details

- Location: San Francisco, California 94109
- Weekly Estimated Pay: $2,520–$2,710
- Start Date: November 2, 2026
- Contract Duration: 13 weeks, ending February 1, 2027
- Schedule: 5 shifts per week, Monday through Friday
- Shift: 8-hour day shifts, approximately 8:00 a.m. to 4:30 p.m.
- Call, Weekend, and Holiday Coverage: None required
- Patient Population: Adult patients
Hospital Pharmacist Job Requirements
- At least 2 years of pharmacist experience, including both inpatient and outpatient pharmacy practice
- Active California Registered Pharmacist (RPh) license
- PharmD and RPh credentials
- Previous transplant pharmacy experience required
- Experience with Epic or comparable electronic health record systems
- Strong knowledge of clinical dosing, pharmacokinetic monitoring, renal dosing, anticoagulation, and high-alert medications
- Experience with sterile technique, IV piggybacks, IV push medications, TPN compounding, and controlled analgesia
- Ability to complete required compliance documentation and onboarding requirements before the assignment start date
Clinical Pharmacist Responsibilities
- Provide safe, effective pharmaceutical care to adult patients in inpatient and outpatient settings
- Review medication orders for accuracy, appropriateness, interactions, contraindications, and therapeutic outcomes
- Perform clinical dosing, therapeutic medication monitoring, pharmacokinetic dosing, renal dosing, cardiac dosing, and anticoagulation management
- Prepare and verify compounded medications, IV piggybacks, IV push medications, TPN mixtures, and other sterile products
- Support transplant-related medication management and specialized clinical pharmacy services
- Monitor high-alert medications and contribute to medication safety initiatives
- Document clinical interventions and medication-related activities accurately in Epic
- Collaborate with physicians, nurses, and other members of the interdisciplinary care team
