Key Responsibilities In this role you will be responsible for: Provides direct patient care as defined in the State of Nursing Practice Act Implements plan of care indicated by the clinician Provides accurate and timely documentation consistent with the plan of care Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care Participates in coordination of home care services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, SLP) to the case manager or Clinical Supervisor Uses equipment and supplies effectively and efficiently Participates in personal and professional growth and development Performs other duties as assigned by the case manager or supervisor Required Qualifications Graduate of an accredited practical nurse or vocational nursing program Current state license as an LPN/LVN 1+ years experience as a nurse in a clinical care setting Demonstrates the ability work collaboratively as part of an interdisciplinary team Excellent verbal and written communication skills Strong organizational and time management abilities Solid clinical judgement and the ability to follow care plans accurately Reliable transportation and valid automobile insurance coverage Desired Qualifications Experience in home health or hospice preferred Location Office Location: 801 Rosehill Road, Jackson, MI 49202 Main Service Area: Lansing and surrounding Hours 8:00 am – 5:00 pm, Monday through Friday Pay Range $35 - $38 USD How We Care for You Minimum of 3 Weeks Paid Time Off (PTO) Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company paid Short Term Disability 401(k) with Employer Match Mileage Reimbursement Company Vehicle Program for field roles Pet Insurance ID and Fraud Protection And more...
