Responsibilities
- Conduct PT patient evaluations and develop individualized treatment plans for residents
- Implement physical therapy interventions including therapeutic exercises, gait training, ROM, strength and balance training, and functional mobility activities
- Monitor progress, adjust treatment plans as needed, and document progress in required records
- Collaborate with an interdisciplinary team to support comprehensive resident care and contribute to discharge planning
- Educate residents, families, and caregivers on home programs, safety, and activity modification; ensure patient safety and infection control practices

Requirements
- Current Physical Therapist license in Maine
- Doctor of Physical Therapy (DPT) degree or equivalent
- Relevant clinical experience in a skilled nursing facility or similar setting preferred
- Ability to work the specified shift and participate in a travel assignment
- Strong communication, documentation, and teamwork skills
