The Clinical Dietitian assesses nutritional status and develops and implements a comprehensive nutrition care plan. Completes nutrition focused physical assessments to accurately diagnose malnutrition. Participates in the interdisciplinary team providing care coordination, education, and support during the discharge process. Provides nutritional counseling, education, and symptom management to clients in classes or individually in the outpatient setting.Under minimal supervision provides medical nutrition therapy (MNT) to individuals and groups for nutrition-related problems, chronic disease management, prevention and well-being.Applies critical thinking to the nutrition care process of nutrition diagnosis, intervention monitor and evaluate the patient's status and the effectiveness in creating a holistic plan of care that includes cultural diversity and special needsProvides specialized nutrition support under supervisionParticipates in interdisciplinary rounds, providing care coordination, education and supportParticipates in quality operating system (QOS) improvement efforts to improve nutrition care provided throughout the organizationProvides nutrition education. Identifies formal and informal learning opportunities to provide appropriate nutrition educationOther duties as assignedQualifications:Registered Dietitian Nutrition (RDN/RD) or must be registration eligible. If registration eligible, must pass the Commission on Dietetic Registration (CDR) national registration exam to achieve RDN status within six months of the date of hire and show proof of application for State of Wisconsin certification within 90 days of RDN status.
