Growing together.Primary ResponsibilitiesProvide physical therapy services to patient according to a written physician's plan of care.Assess and evaluate therapeutic/rehabilitative/functional status, participation in the development of the total plan of care and evaluates home environmentAssesses for muscle strength, mobility, gait, and range of motion - potential for rehabDirects physical therapy treatmentInstructs patients and/or families/caregivers in the use and care of therapeutic appliances.Determines priority needs for physical therapyReports to physician patient's reaction to treatment or changes in conditionInitiates physical therapy program and instructs other personnel and/or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferencesMay train patient in the use of prosthetic device and/or orthotic devicesIdentifies patient and family/caregiver needs for other home health services and refers as appropriatePrepares and submits clinical and progress summaries based on the attainment of goalsParticipates in discharge planning for patientProvides physical therapy consultation to families/caregivers when indicatedProvide in-service education related to Physical Therapy to Agency personnel as neededParticipates in peer consultation processSupervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs firstMaintains contact/communication with other personnel involved in the patient's care to promote coordinated, efficient careMaintains clinical competency in physical therapy practice and theoryWill participate in Agency on-call coverage as outlined in Agency policyThe above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job related tasks other than those stated in this descriptionYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required QualificationsDegree from a baccalaureate or master's program or doctorate in physical therapy approved by an accredited organizationActive unrestricted State of Nevada license to practice as a Physical TherapistHave and maintain current CPR certification2+ years of appropriate experience as a Physical TherapistCommunity/home health experienceExperience working with an electronic medical record applicationDemonstrates thorough knowledge of current Physical Therapy professional standards of care, and state and federal regulationsBasic PC skills to include Window, Outlook and Other email systemsValid Nevada driver's license with automobile that is in good working order and maintain personal auto insurance coverage in accordance with organization requirementsPreferred QualificationsDemonstrate excellent observation, verbal and written communication and organization skillsComplies with accepted professional standards and practicesProven ability to prioritize and communicate objectives clearlyPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 - $130,000 annually based on full-time employment.
