Evergreen Healthcare Group is seeking a compassionate, relationship-driven Clinical Liaison / Respiratory Therapist to support our skilled nursing and post-acute care communities in the Seattle area. In this regional role, you’ll serve as a vital connection between hospitals, families, physicians, and our care teams — helping ensure residents transition smoothly into post-acute care while receiving high-quality respiratory and clinical support. Why This Role Stands Out Competitive salary range: $90,000–$120,000 annually Mileage reimbursement for work-related travel Private healthcare plan (Medical, Dental, Vision) DailyPay – access earned wages before payday Life Insurance & Employee Assistance Program Professional development and ongoing training opportunities Supportive leadership team with opportunities for growth Opportunity to make a direct impact on patient transitions and quality of care Key Responsibilities Develop and maintain strong relationships with hospitals, physicians, discharge planners, case managers, and referral partners Coordinate seamless admissions and transitions from acute care to post-acute/skilled nursing settings Evaluate referrals and assist with clinical screenings and pre-admission assessments Conduct respiratory assessments and monitor residents with cardiopulmonary conditions as needed Administer and oversee respiratory therapies including oxygen therapy, aerosol treatments, tracheostomy care, suctioning, and ventilator support when applicable Collaborate closely with interdisciplinary care teams to develop and implement individualized care plans Serve as a clinical resource for residents, families, and referral sources Maintain accurate documentation in compliance with facility policy and state/federal regulations Support census development initiatives and track referral activity Ensure respiratory equipment is maintained, cleaned, and operated properly Participate in quality assurance and regulatory readiness activities Qualifications Required: Current Washington State LPN, RN, or Respiratory Therapist license RRT or CRT credential preferred for RT candidates Current BLS certification Strong communication, organizational, and relationship-building skills Ability to work independently and travel throughout the Seattle market Preferred: Experience in skilled nursing, long-term care, post-acute care, or hospital discharge planning Experience with ventilator-dependent or tracheostomy residents Background in clinical liaison, admissions, respiratory therapy, or case management At Evergreen Healthcare Group, we believe people always come first.
