Physical Therapist (PT) - Long Term Care (LTC)

Job Description

The Physical Therapist (PT) in Long Term Care is responsible for evaluating, screening, and rehabilitating patients with physical functioning disorders and/or wound care. You will develop and implement appropriate plans of care following regulatory and clinical practice standards.

Key Responsibilities

Effectively screen, evaluate, and rehabilitate patients to address physical functioning disorders.
Utilize standardized assessments and evidence-based practices to guide clinical interventions.
Administer various rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
Assess patient progress and make necessary adjustments to treatments while documenting in clinical records in compliance with regulatory requirements.
Evaluate treatment outcomes for effectiveness and modify services as needed; refer patients based on outcomes.
Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Maintain effective communication with supervisors and healthcare team members concerning patient progress and treatment plans.
Deliver comprehensive discharge summaries of services provided according to regulatory and clinical requirements.
Document all required information in the electronic documentation system and ensure patient records meet company policy.
Accurately record treatment minutes in accordance with regulatory requirements.
Participate in required meetings such as patient care conferences and rehabilitation meetings.
Engage in facility and company trainings, in-services, and conferences as needed.
Facilitate the acquisition of necessary durable medical equipment (DME) or supplies in line with regulatory standards.
Supervise and support physical therapist assistants, rehabilitation aides, and students.
Adhere to the American Physical Therapy Association (APTA) standards and state-required practice acts and ethics throughout professional practice.

Qualifications

Minimum of 1 year of experience as a Physical Therapist.
Experience in Long Term Care (LTC) or Rehabilitation settings is required.
Eligibility for Massachusetts State License.
