Physical Therapist

Assess, plan, and deliver physical therapy services according to physician orders and the State Physical Therapy Practice Act Provide individualized 1-on-1 rehabilitation treatments at Covenant Village and in patient homes throughout Gaston County Evaluate home environments, muscle strength, gait, mobility, and range-of-motion potential Build tailored rehabilitative care plans Educate patients, families, and caregivers on home exercises, safe mobility strategies, and therapeutic appliances Participate in clinical conferences Coordinate integrated care plans with multidisciplinary teams Identify referrals for additional home health PCA services Maintain progress summaries based on goal attainment Supervise Physical Therapist Assistants in compliance with state regulations and company standards Requirements Degree in Physical Therapy from an accredited program Current Physical Therapy license in North Carolina, or eligibility to obtain it before the start date Excellent clinical skills Excellent interpersonal skills Excellent fiscal management skills Proficiency with computers and electronic documentation systems Core Competencies Demonstrates expertise in delivering individualized physical therapy services, developing tailored rehabilitative care plans, and coordinating integrated care with multidisciplinary teams.
