Salary: $80,350.40/Annually Opening Date: 05/28/2026 Closing Date: 06/04/2026 Job Summary To perform a variety of skilled tasks in the implementation and operation of the Nurse‑Family Partnership program, a highly refined and successful approach to long‐established nurse home‑visiting services. Distinguishing Characteristics Nurse Home Visitors must possess a high degree of personal maturity and a strong desire to deliver home‑based preventive intervention to young families, developing therapeutic relationships with each parent in the program. This role extends beyond teaching; nurses become experts fostering the development of young parents across several domains, including health. Supervision Received and Exercised General supervision from the NFP Nurse Supervisor, Director of Health Services, and/or the Assistant Director of Health Services. Important Responsibilities and Duties Target low‑income, first‑time mothers (no previous live births) for enrollment in the Nurse‑Family Partnership program. Ensure enrollment by the 16th week of pregnancy (preferably earlier) and not after the 28th week, continuing visits through the child’s first two years. Follow in‑depth program guidelines focusing on maternal health, quality caregiving for the child, and maternal development. Follow a visit schedule matched to developmental stages of pregnancy and early childhood, adjusting to each family’s needs. Carry a caseload of no more than 25 active families. Maintain a flexible schedule, allowing evening or weekend visitation if necessary. Be involved in the mother’s support system, including family members, the father when appropriate, and friends. Assist families in accessing and using other health and human services they may need. Meet weekly with the NFP Nurse Supervisor for case conferencing and professional development. Use the Nurse Family Partnership data collection and reporting system to track family characteristics, program operation, and progress toward objectives and excellence. Attend required workshops and training sessions, including: Unit1: Fundamentals of NFP Nursing Practice Unit2: The first face‑to‑face session in Denver, CO Unit3: The second face‑to‑face session in Denver, CO Maintain and update records, files, and reports for the Nurse‑Family Partnership program. Perform other related duties as assigned. Qualifications Knowledge of Nursing principles, practices, methods, and techniques of the assigned program area; current trends and developments in the same area. Public health program development principles and techniques. Ability to Maintain an appropriate level of current knowledge regarding assigned nursing and program responsibilities. Establish and maintain cooperative working relationships in the course of work. Communicate effectively, both orally and in writing. Work independently in the absence of supervision. Experience and Training Guidelines Any combination equivalent to experience and training that could likely provide the required knowledge and abilities is qualifying. A typical path includes: Two (2) years of nursing experience; and/or experience in obstetrics, maternal and child health, community health nursing, or public health. A Bachelor of Science Degree in Nursing from an accredited college or university (required). Possession of, or ability to obtain, an appropriate Texas driver’s license.
