Verifies and enters medication orders, proactively communicating with medical staff to resolve problems. Provide direct patient care, including pharmacotherapy assessment, therapeutic monitoring, pharmacokinetic dosing, and transitions‑of‑care. Provides accurate, safe, and timely medication therapy, and promotes cost‑effective drug therapy. Precept, mentor, and educate pharmacy students, residents, and staff, and provide continuing education to interdisciplinary team members.
