Confluence Psychological Services is an outpatient counseling service supporting individuals creating positive change in their lives. Confluence Psychological Services provides therapy, support and guidance for individuals in a community setting which supports their social-emotional growth and guides participants on a journey to explore their passions, values, strengths, and skills, carving a path towards meaning, purpose, and productivity. Confluence Psychological Services provides mental health, psychiatric services, to clients enrolled in services at Confluence Transitions, LLC as well as to young adults seeking similar services from the Burlington, VT community.

The Occupational Therapist at Confluence Psychological Services will manage a caseload of clients enrolled in the Confluence Transitions program. The needs of these clients vary significantly, however, all of the needs fall within the scope of helping young adults develop skills around their activities of daily living, executive functioning, and systems that help our young adult clients develop confidence and capability to live in more independent environments.

Occupational Therapists are responsible for evaluating participants' needs, creating treatment plans, supporting participants in implementing their treatment plans during weekly sessions. Tracking and monitoring progress towards treatment goals and educating participants and their families. An Occupational Therapist interacts with the program by supporting the intervention to help individuals heal, recover and move forward from their challenges.

Responsibilities:

• Weekly individual Occupational Therapy and group sessions with program participants.
• Assessment, Treatment and Discharge Planning.
• Continued assessments and check-in sessions of and for participants.
• Liaises with and supports direct care staff (mentors and coaches) to better the support of participants, the therapeutic process and the intervention model.
• Review medical history, observe daily tasks, and test physical, mental, and cognitive capabilities.
• Create Treatment Plans: Design personalized, goal-oriented programs with specific therapeutic activities and exercises.
• Teach Daily Skills: Help clients relearn everyday tasks like dressing, eating, bathing, and cooking.
• Recommend Adaptations: Suggest improvements and safety modifications for homes, schools, and workplaces, including assistive equipment.
• Educate Families: Train caregivers and family members on how to support and care for the patient safely.
• Track Progress: Monitor and record patient improvements, adjusting care plans as necessary.
• Direct consultation with the clinical and Transitions team and participation in weekly clinical team meetings.
• Correspondence with referring professionals, previous clinicians and other programs/schools.
• Support during site visits by professionals and potential participant visits.
• Participation in marketing events, presentations at conferences.
• Writing and editing for publication, including marketing collateral, websites and blogs.
• Periodic on-call availability to address emergency clinical needs with direct care staff.

Qualifications:

• Education and Licensure requirements for the state of Vermont in the Occupational Therapist professional standards.
• Academic experience and/or professional interest in working with emerging adults.
