Requirements

• Board Eligible / Board Certified in FM or IM (Family Medicine Preferred)
• Completion of U.S. residency from an accredited Family Medicine or Internal Medicine Program
• Commitment to quality patient care
• M.D./D.O.
• Active or ability to obtain a medical license in the state of Georgia
• Active DEA
