RN Part-Time - IMSL Float Pool (Outpatient M-F)

Clinical Staff - Nursing

The RN will float to outpatient primary care clinics within the service line as needed, 8-hour shifts vary from 0730-1800. Part-time position. 20+ hours per week with benefits. Outpatient Monday - Friday. The Registered Nurse will promote wellness; improve outcomes, and self-care within a defined population of patients. The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes.

Minimum Qualifications:

• Bachelor's degree in Nursing, Associate's degree in Nursing, Diploma RN, or Master's degree in Nursing.
• Must be a graduate of an accredited RN nursing program
• Valid Registered Nursing License
• Minimum of 1 year of Registered Nursing experience;
• Basic computer skills.
• Excellent communication skills.
• Strong work ethics and accountability.
• BLS Current

Salary Information:

Commensurate with education and experience
