Family Practice Physician (MD/DO)
Location: Antioch, CA

Job Type: Full-Time or Part-Time

Job Summary & Responsibilities:
Provide comprehensive primary care services to patients of all ages in a community-focused outpatient healthcare setting.

Serve as the Primary Care Provider (PCP) for an assigned patient panel while delivering evidence-based, patient-centered care.

Conduct physical examinations, diagnose acute and chronic medical conditions, and develop individualized treatment plans.

Prescribe medications, order and interpret diagnostic testing, and coordinate specialty referrals when appropriate.

Provide preventive care services, health education, and chronic disease management.

Deliver care through both in-person and telehealth visits as appropriate.

Maintain accurate and timely EHR documentation while ensuring compliance with regulatory and organizational standards.

Collaborate with physicians, nurse practitioners, physician assistants, nurses, behavioral health providers, pharmacists, social workers, and other interdisciplinary team members.

Support quality improvement initiatives, utilization review activities, and ongoing patient care optimization.

Provide clinical oversight and mentorship to assigned Advanced Practice Providers when applicable.

Participate in chart reviews, performance improvement projects, and clinical best practice initiatives.

Manage an average patient volume of approximately 18-22 patients per day.

Provide care to a diverse patient population, including pediatric, adult, and geriatric patients.

Education, Licensure, and Experience:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.

Active California Physician License required.

Current DEA Registration required.

Current BLS Certification required.

Board Certified or Board Eligible in Family Medicine, Internal Medicine, Pediatrics, or Preventive Medicine preferred.

Experience in Primary Care, Family Medicine, Community Health, or Outpatient Practice preferred.

Experience utilizing Electronic Health Records (EHR) systems.

Strong clinical, communication, and interpersonal skills.

Schedule:
Flexible Full-Time or Part-Time opportunities available.

Monday through Friday schedule.

No call responsibilities.
