Responsibilities:

• Provide direct patient care under the supervision of a licensed occupational therapist (ot)
• Implement individualized treatment plans as prescribed by physicians and ots
• Complete daily documentation and weekly progress reports in compliance with state and payer guidelines
• Communicate patient progress and concerns with therapy and nursing teams
• Assist with caregiver and family education for safe discharge planning
• Participate in interdisciplinary team meetings and care planning
• Support restorative nursing programs and train restorative aides
• Maintain a clean, safe, and organized therapy environment
• Report equipment needs and safety concerns promptly
• Participate in ongoing training, in-services, and staff development

Qualifications:

• Active cota license in good standing (required)
• Knowledge of the occupational therapy practice act
• Experience in skilled nursing or post-acute care preferred
• Familiarity with casamba and pointclickcare preferred
• Strong documentation, communication, and interpersonal skills
• Team-oriented with a patient-centered approach
