The Care Team
The Care Team Hospice is seeking a compassionate RN Case Manager to join our Southern Will County team serving Matteson, Crete, Richton Park, and South Holland. You will coordinate patient care, manage plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients at the end of life.
