Job Description

This position requires a skilled professional with a minimum of 2 years of acute hospital social work experience. The selected candidate will demonstrate expertise in assessing patients, care coordination, and providing consult-based services. Ideal candidates will have knowledge of drug and alcohol treatment options, experience working with patients experiencing homelessness, and familiarity with mental health issues or crises. Responsibilities include:

• Organizing and participating in family meetings.
• Participating in the development of treatment plans.
• Providing individualized patient and caregiver education and support.
• Conducting biopsychosocial assessments.
• Documenting care delivery, plans, and patient responses.
• Collaborating with multidisciplinary teams to ensure continuity of care.
• Educating patients and caregivers to facilitate discharge.

Qualifications

• Minimum 2 years of experience in acute hospital social work.
• Valid California state social work license (or pending license accepted).
• Experience with 5150/involuntary holds, adoption/foster care issues, and child protective services.
• Knowledge of emergency department procedures and mental health disorders.
• Familiarity with governmental programs such as Medicaid and Medicare.

Work Environment

This position requires floating as needed to meet departmental requirements. The daily census is variable, and the ideal candidate will be comfortable working with diverse patient age groups, including neonates, adolescents, adults, and geriatrics. The candidate should be adaptable to potentially working in various related units.

Other Requirements

• Must be able to commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
• Must have prior travel experience as this is a traveling position.
• References required covering at least 1 year of work experience within the last 3 years.
