Participate in the development of speech therapy plans of care for patients. Make visits to the patients for initial and ongoing assessments to identify the patient’s level of functioning and ability to communicate, including OASIS Assessments at appropriate time points. Communicate with the physician before the start of care and as needed. Implement the plans of care. Provide education and instruction to team members, patients, and families. Document care and submit paperwork in an accurate and timely manner to update and maintain the patient medical records. Required Qualifications 1 year of experience (preferably in a clinic setting).
